Form Fox

Form Fox is a Discord bot used for creating forms and applications in your server. Whether you need an entry form to verify new members, or would like to create an application for incoming server moderators, this bot is for you!

Features

Multiple forms per server

Servers can create multiple forms of various types, from verification to mod applications to surveys. Currently there are no limits to the number of forms you can make, nor to the amount of responses they can receive!

Question types

Form Fox allows admins to set up multiple types of questions, including multiple choice, text-based, and attachment (image-only) types. You can even make users submit numbers or dates for specific questions!

Highly customizable form embeds

Fox comes with a number of customizable elements for your forms' embeds, including a form icon and banner, button styles, acceptance message, and plenty more! You can find most options under the /form customize set of commands

Answer-based roles

Want to make sure that only adults can receive your server's Adults role? Can do! With Fox, users can be granted roles based on the answers they gave. With commands under /question roles, you can set up your form so that users who said they're over 18 in your entry form will automatically receive the role when their response is accepted
